Community Nursing Care

Flourish Holistic Community nurses are caring and highly skilled professionals and are

always ready to help. Community service nursing exists to meet your clinical and

personal needs so that the participant can live a healthy and an independent lifestyle.

Our community nursing care service is provided in your home or at community clinic by

a Qualified nurse and You can access these services both on regular and occasional

basis. A registered nurse will assess your clinical need for nursing and/or personal care

services. A registered nurse will then determine in consultation with you the type and

frequency of nursing and personal care services you need. 

Flourish Holistic community health nurses advocate for you and help to give you more

of a voice in your own care. Some of the services that our community nurses offer are:

• Wound care

• Stoma care

• Respiratory support

• Continence advice and management

• Catheter care and management

• Tube-feeding assistance

• Dementia management

• Care after a hospital stay

• Monitoring of blood pressure, pulse, temperature, and blood sugar levels

• Education for clients and their families

• Visiting nurse and assessment

• Development of care plans
